Thank you for taking the time to complete this form and enrolling your child in our playgroup.

This information will help us to better plan our playgroup program to suit all the children’s abilities and interests. Please always feel free to ask our Playgroup Co-ordinator any questions you may have and feel assured that all information will remain confidential.

Please read our Playgroup Policy in this form and ensure that you notify us of any changes to this form throughout the year.

Families will:

Agree to abide by the Playgroup Policy.
Be aware that our Playgroup hours are between 10 am – 12 noon. Please adhere to these times.
Agree to not attend playgroup if they or any member of the immediate family have been sick or are currently feeling unwell.
Agree to provide a copy of the child/rens immunisation record before attending playgroup
Be responsible for supervising their own child/ren whilst on the grounds of Wyong Preschool. Whilst we have a Playgroup Co-ordinator her role is to facilitate play rather than supervise the children.
Parents/caregivers are not to use their phone unless in an emergency situation.
Understand that our large playground is designed for the 3 to 6 year age group. Therefore, you need to be vigilant in supervising your child as they could come across uneven surfaces, water play and challenging obstacle courses/equipment. Our larger playground is usually available for our playgroup families to access after 11.30 am.
Be sun safe aware by providing their child/ren with a hat and applying sunscreen as necessary. Sunscreen and spare hats are available upon request.
Provide a healthy morning tea for their child, bearing in mind we are NUT and EGG FREE.
Be able to access morning tea and coffee/tea/cold water set up in the staffroom. Please ensure hot drinks are consumed in a lidded cup, only in the playgroup area, being mindful of children’s safety. When placing your hot drink down please put it safely in the staffroom. It would be appreciated if families washed up their morning tea cups when finished. Please BE AWARE that the kitchen is a child-free zone due to safety requirements.
Be able to use toilets and nappy changing facilities in the amenities area adjacent to the staffroom.
Parent/caregiver must supervise their child when they are using the toilet facilities.
Assist in helping to pack away playgroup by placing equipment near the shed roller door before going out to the big playground with your child.
When children are in the large/main playground, a parent/caregiver must supervise them at all times.
When leaving the large/main playground parents must help their child/ren tidy the areas that they have played in.
Inform Playgroup Co-ordinator if they do not want their child’s photo taken as part of the documentation of your child’s play and learning.
Agree to not share any photos of other children on social media, e.g. facebook
Provide feedback to our Playgroup Co-ordinator or Nominated Supervisor of any ways we can improve upon our playgroup, any concerns you may have. Positive feedback is most welcome.
